In a letter to Universal Orlando Resort Team Members, Karen Irwin, President and COO of the resort, addressed the September 17th tragedy on the Stardust Racers coaster at Epic Universe that led to the death of a Florida man.
In the message, first obtained by WESH 2 News Orlando, Irwin wrote: “Our hearts are with our Guest’s family and loved ones, and with all of you who have been impacted by this tragic loss.”

She went on to share preliminary findings: “Our internal findings to date confirm that the ride systems functioned as intended, equipment was intact at the ride’s start, throughout the duration of the ride and upon the ride vehicle’s return to the station, and our Team Members followed procedures. The attraction remains closed as we continue to work through a comprehensive review process in cooperation with the ride manufacturer of record. Safety is, and always will be, at the forefront of everything we do.”
What Happened?
The Epic Universe roller coaster death incident occurred last Wednesday at approximately 9:20 p.m. on Stardust Racers, Epic Universe’s new dueling roller coaster.

A guest was found unresponsive during the ride and later pronounced dead at a nearby hospital. The victim was identified as 32-year-old Kevin Rodriguez Zavala, who had a pre-existing spinal condition and required a wheelchair transfer to board the attraction.
Both Universal Orlando Resort and the Orange County Sheriff’s Office launched investigations. Out of respect for the family of the deceased, and for safety reviews, operations of Stardust Racers were suspended, and the attraction currently remains closed.
State Investigators Align With Universal’s Findings
On September 22nd, state investigators confirmed they are in agreement with Universal’s internal findings.
According to the Florida Department of Agriculture and Consumer Services: “The department’s current findings align with those shared by Universal after monitoring the same tests and reviewing the same information. The investigation is ongoing, and additional information will be released as it becomes available.”

Meanwhile, civil rights attorney Ben Crump has been retained by the victim’s family. His office issued a statement saying they are seeking “full transparency, including the release of maintenance logs, inspection records, ride data, and video footage, to understand the circumstances surrounding Kevin’s death.”
At this time, no lawsuit has been filed against Universal or the coaster’s manufacturer, Mack Rides.

The company emphasizes a “from practice, for practice” design philosophy, integrating real-world operational expertise into ride development.

Mack has also partnered with TÜV SÜD expert Roman Hauer to ensure all attractions meet or exceed global safety standards. Nearly all major ride components are produced in-house at Mack’s Waldkirch facility, allowing for rigorous quality control.
Despite its longstanding reputation for safety and innovation, the Stardust Racers incident has drawn increased scrutiny to the company’s practices.

Universal remains at the center of attention as the investigation continues.

Irwin closed her letter by encouraging staff to lean on each other during this difficult time.
“In the days ahead, please be mindful that this is an ongoing investigation,” she said. “I ask that you continue to support each other and remember emotional support resources are available for you both in-person and virtually. Thank you for living our values and the commitment you bring to our Guests and each other, especially in moments like this. Together we will continue to move forward with compassion, care and professionalism.”
